Special Education Transition Project
The ESC Region 19 Transition Program supports parents, families, and educators by providing resources and training on federal and state guidelines for transition services. It offers staff development and technical assistance to help districts, charter schools, families, and community agencies guide students toward success as they approach high school graduation and beyond.
For many students, graduating high school brings more questions than answers. For students with disabilities, the future can feel even more uncertain. Transition planning is a structured process designed to equip students with disabilities and their families with the knowledge and support needed to navigate the shift from public school to adulthood.

It also provides helpful information for your parents. This guide has steps you and your parents can take to make sure you are able to find the right work or educational choices for you after high school. It also tells you where to get the services you will need after high school.
